What expenses should I include?

Include your mortgage payment (principal + interest), property tax, insurance, ongoing maintenance reserves, property management fees, and an allowance for vacancy — leaving these out will overstate your real cash flow.

What is positive vs. negative cash flow?

Positive cash flow means rental income exceeds expenses — the property pays for itself and generates profit. Negative cash flow means you're paying out of pocket each month to cover the shortfall.
