The 4Ts Score estimates the probability that a drop in platelet count is caused by heparin-induced thrombocytopenia (HIT), a serious immune reaction to heparin, helping guide decisions about stopping heparin and further testing.
What does a low 4Ts Score mean?
A low score (0-3) has a strong negative predictive value, meaning HIT is unlikely, around a 5% probability, and heparin can often be continued while other causes of thrombocytopenia are investigated.
What does a high 4Ts Score mean?
A high score (6-8) suggests a high probability of HIT, generally prompting immediate discontinuation of heparin and starting an alternative anticoagulant while confirmatory lab testing is pending.
