What is the Braden Scale used for?
The Braden Scale assesses a patients risk of developing pressure ulcers (bedsores), commonly used in hospitals and long-term care to guide preventive interventions like repositioning schedules and pressure-relieving surfaces.
Why does a lower score mean higher risk?
Unlike many clinical scores, the Braden Scale is scored so that lower numbers indicate more severe impairment in each category, so the lowest possible total score (6) represents the highest pressure ulcer risk.
What score indicates high risk?
A total score of 12 or below is generally considered high risk, typically prompting more intensive preventive measures such as frequent repositioning and specialized support surfaces.
