Why does calcium need to be corrected for albumin?

About 40% of blood calcium is bound to albumin. When albumin is low, total measured calcium looks artificially low even though the biologically active, unbound calcium may be normal. Correcting for albumin gives a more accurate picture.

What albumin level is considered normal for this formula?

The formula assumes a normal albumin of 4 g/dL as the reference point, adding 0.8 mg/dL of calcium for every 1 g/dL that albumin falls below that.

When should ionized calcium be used instead?

Ionized calcium, measured directly, is generally more accurate than any corrected-calcium formula, especially in critically ill patients, but it requires a specific blood gas analyzer not always readily available.
