What is the anion gap used for?
The anion gap helps classify the cause of metabolic acidosis, distinguishing high anion gap acidosis (such as lactic acidosis or ketoacidosis) from normal anion gap acidosis (such as from diarrhea or certain kidney conditions).
Why does albumin matter for the anion gap?
Albumin is a major unmeasured anion in blood. Low albumin can make the anion gap appear falsely normal even when there is a true high anion gap acidosis, so the albumin-corrected value is often more accurate.
Should potassium be included in the calculation?
Most clinical practice uses the anion gap without potassium, but including it is also valid and simply shifts the normal reference range higher by about 2 to 4 points.
