Why does high glucose affect measured sodium?
High blood glucose pulls water out of cells and into the bloodstream, diluting the measured sodium concentration even though total body sodium has not actually changed.
Which correction formula does this use?
This uses the modern Hillier (1998) formula, which adds 0.024 mEq/L of sodium for every 1 mg/dL that glucose exceeds 100 mg/dL. An older Katz (1973) formula used a smaller factor of 0.016 and is less commonly used today.
When is this correction most useful?
It is most useful in patients with significant hyperglycemia, such as diabetic ketoacidosis or hyperosmolar hyperglycemic state, where the true sodium status can otherwise be masked.
