What is the urine anion gap used for?
The urine anion gap is used, alongside the serum anion gap, to help distinguish between gastrointestinal and kidney causes of a normal-anion-gap metabolic acidosis.
Why does a positive vs. negative result matter?
A positive urine anion gap suggests the kidneys are not excreting enough ammonium, pointing toward renal tubular acidosis. A negative result suggests ammonium excretion is intact, pointing toward a non-kidney cause like diarrhea.
Does timing of the urine sample matter?
Ideally, urine electrolytes should be measured on a sample collected over 24 hours to account for normal daily variation, though a spot sample is sometimes used for a quicker estimate.
